CAG HOME
In Denmark and globally, healthcare systems are facing a significant challenge due to the ’double demographic burden, which is characterized by an ageing, co-morbid population, and a shrinking healthcare workforce.
Fortunately, there is a shared understanding of the situation among key stakeholders exemplified by the Clinical Academic Group (CAG) initiative and the recent Health Care Reform (2024). The importance is highlighted of transitioning to a ’Hospital-at-Home’ model as a central strategy to relieve pressure on the national healthcare system.
This CAG-proposal has the following goals:
Short-term:
• Implement evidence-based digital health technologies for diagnostics, treatment, and monitoring at home.
• Transform working culture of healthcare providers to incorporate home-delivery of health care.
• Adjust and apply technologies and workflows together with users and family members.
Long-term:
• Establish evidence supporting the transfer of a substantial part (>10%) of health care from the inpatient to the outpatient settings including health-economic considerations.
• Enhance collaboration between different sectors of the healthcare systems establishing a dedicated and focused relation between hospitals and primary health care.
• Develop sustainable novel technologies and modalities, integrating technological healthcare delivery to empower patients and enable hospital at home models.
The Key Persons bring unique experience in conducting translational and multidisciplinary research, including the development, validation, and implementation of novel technologies for home-based healthcare services. The CAG includes all 4 partners and collaborates with several other CAGs and will be a place for research talents to mature and lead. It aligns with the global trend shifting healthcare from traditional in-hospital settings to home-based treatment, diagnostics, monitoring, and care.
